Application forms

Choose the form corresponding to your own case. There are different forms depending on the applicant's nationality, the level (1st/2nd cycle) of the chosen degree programme, and type of institution (University/ Afam).

You are kindly requested to fill out your application forms in block letters.

Eu national and equated international citizens (to be updated)

Check if you belong to this category (go to Eu and Eu-equated citizens).
In the adfirmative, make your choice depending on the cycle and type of programme of your interest.

1. If you apply to a university, select the right form (Modello) from the set marked by the letters A/ B/ C, etc.; choose:
   • Modello B - enrollment in a CL (1st cycle), or a CLSu/CLMu (2nd cycle long degree in dentistry/medicine/etc.);
   • Modello G - enrollment in a CLS/CLM (all subjects other than those of CLSu/CLMu) or in a CMU1 (both 2nd cycle).
2. If you apply to an Afam institution, go to forms A bis / B bis / C bis, etc.; choose:
   • Modello B bis - enrollment in a CDA1;
   • Modello G bis - enrollment in a CDA2.

Go to "Types of Programmes and Degrees" in the section "Italian Higher Education" to check the meaning of above abbreviations of the names of degree programmes.

Non-Eu citizens living outside Italy

Make your choice depending on the cycle and type of programme of your interest.

1. If you apply to a university, select the right form (Modello) from the set marked by the letters A/ B/ C, etc.; choose:
   • Modello A for pre-enrollment in a CL (1st cycle), or in a CLSu/CLMu (2nd cycle long degree in dentistry/medicine/etc.);
2. If you apply to an Afam institution, go to forms A bis / B bis / C bis, etc.; choose:
   • Modello A bis for pre-enrollment in a CDA1;
